Use this form if you require your PIN number to access your details on the international Choi Kwang Do database at www.choikwangdo.com  Please note, you will need your student number as shown on the front of your licence to enter the site, and as a security check before your PIN will be issued.                                                                          If you do not have this number, please speak to your Chief Instructor
